Long Term Medical Conditions

You need to complete the relevant forms below and any medication and return to the school office as soon as your child is diagnosed with any long term medical conditions that require medicines or medical intervention, such as inhalers, epi-pens or epilepsy medication.

Medication needs to be clearly labelled, and in the original prescription packaging. 

Please ensure you keep these forms up to date with any changes send into the school ASAP.

Short Term Medications

You need to complete the forms below and along with any medication, return to the school office to enable to school to administer any prescribed medication. We are able to administer medication where it is prescribed to be taken 4 times or more during a 24hr period.

We ask where possible that you administer medicines outside of school hours.
